01 / RESEARCH SUMMARY
Cagrilintide Research Summary
Cagrilintide is a long-acting analog of amylin, a hormone released alongside insulin that promotes fullness and slows stomach emptying. It acts on a different appetite pathway than GLP-1 drugs, which is why it is studied in combination with semaglutide (CagriSema). On its own, 4.5 mg weekly produced about 10.8% weight loss at 26 weeks in a phase 2 Lancet trial. Combined with semaglutide in the REDEFINE 1 trial, it produced about 20.4% weight loss at 68 weeks.

Why Cagrilintide draws research interest
These are the areas researchers and the peptide community explore Cagrilintide for most often. They describe what is being studied — not proven benefits, approved uses, or promised results.
Appetite & fullness
Mimics amylin to increase satiety and slow stomach emptying.
Pairing with semaglutide
Studied as CagriSema, which reached about 20% weight loss.
Weight management
About 10.8% weight loss on its own in a phase 2 trial.
Evidence ranges from early lab work to large clinical trials depending on the use — see the sections below for the data and sources.
03 / DOSING & RECONSTITUTION
Dosing & Reconstitution Guide
A single practical dilution with accurate dosing, step by step
Standard Approach (1 mL = 10 mg/mL)
- Reconstitute: Add 1 mL bacteriostatic water to one 10 mg vial → final concentration 10 mg/mL (10,000 mcg/mL).
- Typical range: 0.25–4.5 mg 1x daily · 1–2x weekly (split dose), As long as needed.
- Easy measuring: At 10 mg/mL, 1 unit ≈ 100 mcg on a U-100 syringe, so doses read at 2.5–45 units.
- Storage: Lyophilized: store at −20 °C (−4 °F); after reconstitution, refrigerate at 2–8 °C (35.6–46.4 °F) and do not freeze the mixed solution.
| Protocol | Dose range | Frequency | Cycle | Time off | Half-life | Fasted | Route |
|---|---|---|---|---|---|---|---|
| Injectable · Subq | 0.25–4.5 mg | 1x daily · 1–2x weekly (split dose) | As long as needed | As needed | ~7–8 days (159–195 hrs) | No | Subq |
| Obesity · Phase 2 (2021) | 0.3 / 0.6 / 1.2 / 2.4 / 4.5 mg | Once weekly | 26 weeks | Subcutaneous (study formulation) |

Storage Instructions
Lyophilized (powder)
Store in the freezer at −20 °C (−4 °F) for long-term storage, or refrigerated at 2–8 °C for shorter periods. Keep dry, sealed and away from light. Let a frozen vial reach room temperature before opening to avoid condensation.
Reconstituted
Refrigerate at 2–8 °C (35.6–46.4 °F) and use within about 28 days. Do not freeze. Keep away from heat and direct light, and avoid repeated temperature swings. Swirl gently — never shake.
Reconstitution steps
- Wipe the vial stopper with an alcohol swab.
- Draw the BAC water and release it slowly down the inside wall of the vial.
- Swirl or roll gently until fully dissolved — don't shake.
- Label with the date and concentration and refrigerate.
06 / RESEARCH FINDINGS
What Research Shows
Human research
- A randomized phase 2 trial assessed body-weight reduction in adults with overweight or obesity without diabetes.
- Long-acting amylin receptor activity is a metabolic research target; combination studies must be evaluated separately.
Research context
The user-provided Cag mapping does not independently confirm the vendor product's identity or pharmaceutical equivalence. Trial results apply to the controlled study formulation.
07 / MOLECULAR BIOLOGY
Mechanism of Action
Investigational mechanism
Amylin receptor signaling
A long-acting amylin analog studied for signaling involved in satiety and food intake. Its monotherapy findings are distinct from combination-treatment findings.
Adamantane & the blood–brain barrier
Chemically attaching an adamantane group to a peptide can increase its lipid (fat) solubility. Researchers hypothesize that this modification may help certain compounds cross the tightly regulated blood–brain barrier. The effect depends on the specific modified molecule; greater lipid solubility alone does not establish brain delivery or clinical benefit. This does not apply automatically to the unmodified peptide and is not a reconstitution additive.
08 / LIFESTYLE FACTORS
Lifestyle Factors
- Protein: aim for adequate protein (about 0.7–1 g per lb of goal body weight) to preserve muscle during weight loss.
- Resistance training: lift 2–4x per week to protect lean mass.
- Hydration & fiber: drink plenty of water and eat fiber to reduce constipation and nausea.
- Meal size: eat smaller meals and stop at fullness; greasy, heavy meals worsen GI side effects.
- Titrate slowly: stay at a dose until side effects settle before increasing.
09 / BENEFITS & SIDE EFFECTS
Potential Benefits & Side Effects
Potential Benefits
- Significant reduction in appetite and food noise
- Clinically meaningful weight loss
- Improved blood sugar and insulin sensitivity
- Improvements in blood pressure, lipids and liver fat
Side Effects
- Nausea, vomiting, diarrhea or constipation (most common, usually during dose increases)
- Reduced appetite to the point of under-eating
- Fatigue and occasional headache
- Increased heart rate (especially triple/glucagon agonists)
- Rare: gallbladder problems or pancreatitis — seek care for severe abdominal pain

11 / SAFETY
Safety considerations
- Verify the active identity, lot number, and matching certificate of analysis. Testing does not by itself establish sterility or suitability for human use.
- Follow documented product-specific storage and preparation requirements; stability and diluent compatibility depend on the formulation.
- Research powders are not interchangeable with prescription formulations. Do not infer an injection or nasal protocol from a concentration calculation.
- For medical care, pregnancy, existing conditions, or medication interactions, consult a qualified healthcare professional.
